Our wide variety of locations include: Minneapolis, Hopkins, Hastings, Burnsville, Apple Valley, Dassel, Montevideo, and Fergus Falls. Augustana Care’s Home Transitions program partners with realtors to help elders prepare their homes for sale and guarantee the sale of a home within 150 days—or buy it outright.
